Inspector Notes

A complaint was received in the licensing office on May 3, 2023 regarding toilets not flushing in the Preschool classroom. On May 17, 2023, an on-site investigation was conducted. Interviews were conducted and observations were made. Based on the preponderance of evidence gathered, violations were cited.

Based on staff records review, 2 of 2 records, the center did not obtain required documentation of central registry results and sworn disclosure statement for staff.
1. Staff #1 date of employment 3/28/2022, did not have central registry results in record available for review.
2. Staff #2 date of employment 5/31/2022, did not have sworn disclosure statement in record available for review.
Plan of Correction: All background checks have been reviewed for completion and expiration.

Based on staff records review, 2 of 2 records, the center did not obtain documentation each staff member submitted documentation of a negative tuberculosis screening.
Evidence:
1. Staff #2 (date of employment 5/31/2022) did not have documentation of a negative tuberculosis screening in record.
Plan of Correction: All background checks have been reviewed for completion and expiration.
Standard 8VAC20-780-320-B
Based on observation and interviews, the center did not ensure each restroom area provided for children had toiles that are flushable.
Evidence:
1. In the preschool bathroom, toilets were operable and flushable for access to children.
Plan of Correction: Toilets with issues have been removed and replaced. As soon as a toilet is clogged management unclogs it/places work orders to have plumbers on site. These toilets have been replaced with larger ones and step stools have been provided for easier access.
