Standard 22.1-289.035-B-2
The center must obtain a completed national criminal background check prior to the employee's first day of employment. Staff #1 is currently working and has a completed national criminal background check dated 11 days after their first day of employment.
Plan of Correction: Staff will have background checks done prior to hire.
Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #4, who had been attending for 7 months, did not have documentation of an immunization record on file.
Plan of Correction: Will complete and communicate all kids immunizations records and files required on time.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #2, who had been enrolled for two months.
Plan of Correction: Will ensure children have physical examinations within 30 days. Will remain consistent in checking in with families to ensure completion.

The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children. Staff #1 is currently working and has a TB screening that was dated 2 months after their first day of employment.
Plan of Correction: Will ensure all staff have negative TB test results prior to start date.

Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff #1 who have been actively working at the center for the past 3 months, did not have completed reference verifications on file with date of contact, signature of person who completed the reference check, and the relationship of reference for Staff #1.
