Inspector Notes

This inspection was conducted by licensing staff both on-site and using an alternate remote protocol, including telephone contacts, documents review, interviews and a tour of the program.

A complaint investigation was initiated on 6/29/21 and concluded on 12/2/21. A complaint was received by the department on 6/29/21 regarding allegations in the areas of children's injuries and supervision. A second complaint was received that same day regarding an allegation in the area of behavior guidance. The center director was contacted by telephone to conduct the investigation. Interviews with staff were conducted both on-site and by telephone. Documentation related to the allegations was also reviewed. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
