Inspector Notes

This inspection was conducted by licensing staff both on-site and using an alternate remote protocol, including telephone contacts, documents review, interviews and a tour of the program.

A complaint investigation was initiated on 7/28/21 and concluded on 12/2/21. A complaint was received by the department on 7/20/21 regarding allegations in the areas of children's injuries and supervision. During the course of the investigation, a second complaint was received regarding supervision. The center director was contacted by telephone to conduct the investigation. Interviews with staff were conducted both on-site and by telephone. Documentation related to the allegations was also reviewed. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
