Inspector Notes

An unannounced monitoring inspection was initiated and completed on May 30, 2024, in response to two self-reports received by the licensing office on April 2, 2024, with allegations regarding children?s injuries and another self-report received on April 17, 2024, with regards to supervision by staff. The inspector reviewed 2 children?s records and 5 staff records. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
