During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 3/19/25. The on-site inspection began at 11:10am and ended at 3:30pm. The inspector reviewed compliance in the areas listed above. There were 75 children present and 16 staff. The inspector reviewed 7 children's records and 7 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Areas of the center shall be maintained in a clean and safe condition.

In one of the two discovery preschool bathrooms, there were two rusted screws with sharp points sticking out of the wall above a toilet. There were also multiple areas of rust on the bathroom wall above each of the toilets.
Plan of Correction: The center responded with the following: Management will submit a work order today to have the rusted screws and rust on the walls removed.

Children?s hands shall be washed with soap and running water after toileting.

Staff 1 changed two children's diapers in the infant room and did not wash either child's hands with soap and running water.
Plan of Correction: The center responded with the following: Management will have a meeting with all staff to discuss diapering and handwashing procedures.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for child 1, enrolled for over one year, and the record for child 2, enrolled for one month, does not contain the work phone number for one parent. The record for child 3, enrolled for over two years, does not contain the place of employment and work phone number for one parent.
Plan of Correction: The center responded with the following: The parent of each child was contacted during the inspection and the missing information was obtained and documented in their records.
