During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced complaint inspection and investigation was initiated on 1/13/25 and was completed on 4/11/25 in response to a complaint received by the licensing office on 1/13/25 relating to allegations in the area of staffing and supervision. The Department requested that the provider complete an Intensive Plan of Correction (IPOC) as a result of an inspection on 5/30/24 related to staffing and supervision. The inspector reveiwed 1 children's record on-site on 3/19/25. This inspection included document review, a tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/22/25. A POC submitted after this date will not appear on the public website.

Repeat Violation
The required ratio for 2 year old children is one staff per eight children. The 2 year old classroom has been over the ratio of 1 staff per 8 children on multiple days during the past six months. The amount of time the classroom was out of ratio varies from 1-2 minutes up to approximately 20 minutes at a time.
Plan of Correction: The center responded with the following: Schedules have been updated to ensure adequate staffing. Center director will partner with district leader if ratio cannot be maintained.

The required ratio during the designated rest period for children two years of age is one staff per sixteen children. On 3/27/25, staff 1 was observed to be alone and supervising 22 two-year old children in the classroom during the designated rest period for approximately 15 minutes. Documentation showed that the two-year old classroom was frequently over the ratio of 1 staff per 16 children during nap time for approximately 30 minutes at a time for a period of two to three weeks.
Plan of Correction: The center responded with the following: Schedules have been updated to ensure adequate staffing. Center director will partner with district leader if ratio cannot be maintained.

A written and signed agreement between the parent and the center shall be in each child's record by the first day of the child's attendance. Documentation of the written parental agreements was not signed by the parent and placed in child 1's record until 16 days after the child's first day of attendance.
Plan of Correction: The center responded with the following: Moving forward, all enrollment agreements will be signed by management and family prior to first day.
