During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection and investigation was initiated on 5/19/25 and completed on 5/23/25 in response to complaints received by the licensing office on 5/1/25 and 5/19/25 relating to allegations in the area of staffing and supervision, children's injuries, toileting procedures and behavior guidance. This inspection included document review, a tour of the facility, interviews and observations. The preponderance of evidence gathered during the investigation supports the allegation related to staffing; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 6/9/25. A POC submitted after this date will not appear on the public website.

Repeat Violation
The required ratio for birth up to 16 months is one staff per four children. The infant classroom was over the ratio of 1 staff per 4 children on 4/28/25, 4/29/25, 4/30/25, 5/1/25 and 5/2/25. The amount of time the classroom was out of ratio on these dates varies from 5 minutes up to 17 minutes at a time.
Plan of Correction: The center responded with the following: CD/AD/3rd Key held staff meeting on 5/1/25 to retrain on ratio compliance, center communication standards with use of walkie talkies and importance of safety and supervision for all children. CD/AD monitoring ratios throughout the day and following up with teachers on awareness.

The required ratio for 16 months up to 24 months is one staff per five children. The toddler classroom was over the ratio of 1 staff per 5 children on 4/28/25, 4/30/25 and 5/1/25. The amount of time the classroom was out of ratio on these dates varies from 28 minutes up to 1 hour and 35 minutes at a time.
Plan of Correction: The center responded with the following: CD/AD/3rd Key held staff meeting on 5/1/25 to retrain on ratio compliance, center communication standards with use of walkie talkies and importance of safety and supervision for all children. CD/AD monitoring ratios throughout the day and following up with teachers on awareness.
