Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A (monitoring/renewal/complaint) inspection was initiated on 12/03/2021 and concluded on 12/06/2021. The director was contacted by telephone to initiate the inspection. There were 42 children present and 11 staff. The inspector emailed the directo a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a record review the center did not ensure all staff had the required repeat background checks 5 years after the last one.
Evidence:
Staff #2's central registry check was dated 04/14/2016.
Plan of Correction: This was an oversight. The background check will be sent for immediately.
