Inspector Notes
A monitoring inspection was conducted on 05/12/2023 in response to a self report received in the licensing office on 04/15/2023 in the area of Programs. A tour of the facility was conducted, in addition to a record review and interviews with staff. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

3Standard 8VAC20-780-400-A
Based on observation the center did not ensure staff interacted with children and one another to provide needed help comfort and support to the children in care.
Evidence:
Staff #1 and Staff #2 were observed during diapering and the transition from lunch to nap.
1. Some of the children were observed walking on cots, crying and making loud noises.
2. Neither staff member verbally interacted with the children or each other.
3. Neither staff member was observed comforting any of the crying children.

Plan of Correction: The center has a new director who observed the classroom during this time. The director will be making some changes in the classrooms and retraining staff on behavioral guidance.

Based on observation the center did not ensure children were not roughly handled.
Evidence:
In the 2 year-old room, Staff #1 was observed abruptly picking children up. roughly placing them on and off the changing table during diapering.

Based on observation the center did not ensure handwashing requirements were met.
Evidence:
Staff #1 did not wash her hands before or after changing children's diapers or handing food or the children's hands after diapering.
1. Staff #1 was observed changing multiple children's diapers wearing the same pair of gloves between changes.
2. With the same gloves worn during diaper changes, Staff #1 handled plates of food in the classroom and opened cabinets. The gloves were not removed and hands were not washed.
