Inspector Notes
The licensing inspector conducted an unannounced monitoring inspection on 05/31/2023 In response to a self-report that was received in the licensing regarding allegations in the area of Supervision. Interviews were conducted with staff, records reviewed and observations of the program were made relating to the allegation. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

4Evidence:
1. On 05/23/2023 a 2 year-old child #1 was found alone and crying after being left outside on the playground and separated from her class for approximately 10 minutes. Staff #3, a teacher from another classroom discovered the child unsupervised when she heard the crying through an opened window in her classroom.
2. During the inspection at approximately 3:00pm, child #2, who is ten years old and has diagnosed developmental delays documented in his file, went to the water fountain located outside of the classroom, walked down the hallway to the front office, knocked on the window of the office and attempted to enter the staff lounge/storage room before the program director intervened and returned him to his classroom. Classroom staff (staff 2) did not notice his extended absence from the classroom nor check on him to ensure his safety.
The director will review children's files to see if there are any other children with documented developmental delays of special needs to ensure the center can provide the best care to the children.
Evidence:
On 05/23/2023 a 2 year-old child #1 was left on the playground unsupervised for approximately 10 minutes. A staff member from a separate class heard crying coming from the playground when she discovered child #2 outside with no staff supervision.
Evidence:
During the inspection at approximately 3:00pm, Staff #5 sent child #2 to the water fountain located outside of the classroom. Instead of returning to his classroom, child #2 walked down the hallway to the front office without any staff supervision. He knocked on the window of the office and attempted to enter the staff lounge/storage room before he was discovered by the program director who returned him to his classroom.
Evidence:
1. The center's supervision policy requires staff to perform and document on a "Child Supervision Record (CSR), that they did a face-to-name check of each child in their care every 30 minutes and/or during any large class transitions.
2. On 05/23/2023 the center did not follow their own policy when staff #1 and #2 did not document having done any face-to-name checks for child #1 who entered the Discovery Preschool classroom at 4:35pm. As a result, Child #1 was left unsupervised on the playground for approximately 10 minutes between 5:00pm-5:10pm.
