An investigation was initiated on 09/16/2024 and completed on 10/01/2024 in response to a self-report received by the licensing office on 09/16/2024 relating to Programs. This inspection included: document review and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Threats such as verbal remarks which are demeaning to the child are forbidden.
Three staff witnesses reported another staff called a year-old curse words that are considered derogatory and threatened to beat the child.

Plan of Correction: An internal investigation was conducted and incident was reported to licensing. The staff was placed on administrative leave until investigation is over.
