Inspector Notes

An unannounced monitoring inspection was completed on 07/21/2025, as a part of the licensure period. The on-site inspection began at 10:00am and ended at 11:20am. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-240-A
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment. Staff #3 did not have documentation of having completed the required 10 hour orientation sponsored by the Virginia Department of Education.
Plan of Correction: The file for Staff #3 will be reviewed and all missing components will be added to the record. The staff will complete the 10 hour pre-service training
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a safe condition. The exterior wooden paneling in the children's play area had chipping paint, 2 protruding screws and 1 protruding nail exposed and in reach of children. In the infant room one of the cribs had a mattress sheet with a whole in it.
Plan of Correction: The director will place a work order to have the exterior paneling replaced and hardware removed. The crib sheet will be replaced.
Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99. The playground's composite play structure did not have the minimum required 9" of resilient surfacing in the equipment's fall zones. There was about 4 inches of mulch.
Plan of Correction: Facilities department had mulch delivered to one playground and not the other. The PD and DM have contacted facilities again to have the mulch filled as soon as possible.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the
center's activities, services, and facilities are maintained in compliance with these
standards and the terms of the current license issued by the department.

The terms of the center?s license include a stipulation that reads ?Only in classrooms #4 and #5 may children under 2 ? years of age be provided care.? The stipulation is to ensure the center is in compliance with the certificate of occupancy issued by the City, which says children under 2 ? are to be cared for in a room with direct egress. The center had a divider that split room #4 into a separate infant and toddler room, resulting in no direct egress in the toddler room. Technical assistance was provided to the center on 01/21/2025 regarding the direct egress requirement.
Plan of Correction: A door that leads to the outside was installed in the toddler half o the room so that they have direct egress.
Standard 8VAC20-780-500-B
Disposable diapers shall be disposed in a leakproof or plastic-lined storage
system that is either foot-operated or used in such a way that neither the staff
member's hand nor the soiled diaper touches an exterior surface of the storage
system during disposal. Staff in the toddler room did not dispose of a child's soiled diaper in the foot operated trash can, rather the soiled diaper was laid next to the child and his clean clothes on the table until she was finished.
Plan of Correction: The director will retrain staff on proper diapering and handwashing in the classroom.

Each staff record shall have all required components in the record.
Staff #3 did not have documentation in the record of her title or hire date.
