Inspector Notes

An unannounced, on-site monitoring inspection was initiated on August 21, 2024 and
completed on August 21, 2024, as a part of current licensure period. The on-site inspection began at 9:50am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 110 children present and 16 staff. The inspector reviewed 10 children?s records and 5 staff records on-site.

This inspection included:
? document review (Injury logs, policies and procedures, emergency drill logs, authorization forms),
? tour of the facility,
? interviews,
? observations of children having snack, playing outside, circle time, reading stories.

Information gathered during the inspection determined non-compliance with applicable standards or
law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-60-A
Based upon review of ten children's records and staff interview, the facility has not ensured that each child's record includes documentation of the child's first date of attendance.

Evidence:
1. The record provided for all ten children did not include documentation of each child's first date of attendance.
2. Assistant Director verified that the records did not have the first date of attendance documented.
Plan of Correction: The first dates of attendance were added during the inspection. All children's records will be checked to ensure that the first dates of attendance are recorded going forward.
