During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on August 28, 2025 and completed on August 28, 2025, as a part of the current licensure period. The on-site inspection began at 8:30am and ended at 10:30am. The inspector reviewed compliance in the areas listed above. There were 101 children present and 19 staff. The inspector reviewed 4 child files and 5 staff files on-site. This inspection included: document review, postings review, tour of the facility and playgrounds, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 9, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On August 28, 2025, Staff 4 was observed outside of the class, in the hall approximately 25 feet away from the door, leaving 8 children alone in the classroom for approximately 3-5 minutes.
Plan of Correction: Spoke to staff about not stepping out of class to greet children when other children are in the classroom. Greeter will escort.

Children under 10 years of age shall always be within actual sight and sound supervision of staff.

During the inspection, three year old children were left alone while staff 4 went down the hallway to assist getting a crying child into the classroom.
Plan of Correction: Spoke to staff about not stepping down to hallway to greet children-they will be escorted by greeter.

The licensee will follow the terms of the current license issued by the department.

The current licensed age range is 3 years to 12 years. There were 4 children in attendance, since August 18, 2025 that were under the age of three. The youngest child is currently 22 months old.
Plan of Correction: Annely Carver, Founding Director, will request to modify license to include 2 1/2 year olds. Email sent to request on 8/29.
