Electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.
Three outlets in 2 rooms did not have protective covers on the outlet. The children were not in the classrooms at the time.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.
On February 26, 2026, Staff 2 was observed in the hallway, approximately 20 feet away from the door of the classroom, leaving 10 children out of sight and sound supervision.

Plan of Correction: All staff will be retrained on sight and sound supervision. This will include not walking out of the classroom for an item in the closet.

SYSTEMIC DEFICIENCY
The required ratio for children ages 3 years to school-age is one staff per ten children.
Staff 6 was observed leaving the classroom when there were 11 children present, to walk to the office to show the director the day's art project. There was another staff member in the room with the 11 children.
