Inspector Notes

An unannounced monitoring inspection was completed from 10:30 until noon. There were 25 children and 7 staff present. The director was also present.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 staff records and 5 children?s records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violation were documented on the violation notice issued to the program.

Based on record review, the center did not have the required out of state background checks completed.
Evidence:
1. Staff # 5 had lived out of state in the last 5 years as indicated on the sworn statement, and did not have an up to date central registry check for that state.
Plan of Correction: An out of state central registry check will be completed.
