An unannounced joint inspection was conducted with the local department of social services to investigate a self-reported incident regarding behavioral guidance and a forbidden action. The onsite inspection was conducted September 22, 2022 from 12:00 noon - 2:00 pm. Based on evidence obtained through multiple interviews and a review of documentation, the self-reported incident was substantiated. A violation notice was issued as a result of this inspection. Based on staff interviews, the facility failed to provide behavioral guidance that is constructive in nature, age and stage appropriate, and intended to redirect children to appropriate behavior. Evidence: on 9/13/2022 between approximately 3:00-3:30 pm, staff #1 and staff #2 repeatedly yelled at child #1 (age 4) in the restroom, when the child's urine accidently got on the wall. Staff reportedly yelled, "You peed on the wall! You peed on the wall!" Child #1 was crying and stating "I did not pee on the wall."
Plan of Correction: Involved staff were retrained on 9/23/22, and on 10/10/22 the entire staff was retrained on acceptable behavioral guidance, appropriate voices, and appropriate touches to use when needed to redirect children.
Standard 8VAC20-780-410-7
Based on staff interviews, it was determined that staff used a forbidden action - punishment for toileting accidents. Evidence: on 9/13/2022 between approximately 3:00-3:30 pm, staff #2 grabbed child #1 (age 4) by the wrist, placing the child's hand on a wall that was wet with urine, and wiping the child's hand all over the urine while yelling, "You peed on the wall! You don?t pee on the wall!"
Plan of Correction: Staff #2 was terminated.
