Inspector Notes

An unannounced monitoring inspection was conducted today with the director from 8:50 am to 11:05 am. There were 58 children present with 9 staff supervising, the ages of the children ranged from 4 months to 9 years. The children were observed finishing breakfast, which consisted of cereal, peaches and milk. The 4 year old children were singing songs while many of the infants were on the floor with staff.
Five staff and six children's files were reviewed during this inspection as well as medication for 3 children and 3 allergy care plans. A sample size of five injury/accident reports were reviewed as well. The areas of non compliance are outlined on the violation notice.

Hazardous substances shall be kept locked.
Seven containers of disinfectants and cleaning agents were stored in an unlocked closet in the Transitional Kindergarten classroom.
Plan of Correction: Locked immediately and checked more frequently.
Standard 8VAC20-780-40-M
A current written list of all children's allergies, sensitivities, and dietary restrictions shall be in each room.
Child A has an egg and lentil allergy as documented in the file. The eggs were identified as a sensitivity on the list and the lentils were not included on the written list.
Plan of Correction: Parent updated paperwork that day.
Standard 8VAC20-780-510-P
When authorization for medication expires, the parent shall pick the medication up within 14 days or the parent must renew the authorization or the medication shall be disposed of by the center.
The parent and physician authorization for an emergency medication for child B expired on 3/10/24. The emergency medication had not been renewed, picked up or disposed of.
Plan of Correction: Appointment had already been scheduled, all was updated since visit.
Standard 8VAC20-780-60-A-8
A written care plan for each child with a diagnosed food allergy by a physician is required.
There was no written care plan on file for the egg and lentil allergy for child A.
