Inspector Notes

A renewal inspection was conducted on September 22, 2022 from 8:25am to approximately 10am. There were 21 children present and five staff members. A sample size of 8 records were reviewed which included five children?s records and three staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during group activity, free play, and story time. First aid kits and emergency preparedness documents were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and a violation is documented. The violation is listed on the violation notice issued to the center and were reviewed with the site owner at the exit interview.

Based on record review and interview, it was determined that the center did not ensure that each child enrolled contain the work phone number of each parent who has custody.
Evidence:
1. The record for Child #1 did not contain the work phone numbers listed for both the mother and father.
2. Staff #1 confirmed that the record for Child #1 did not contact the work phone numbers for both parents.
Plan of Correction: The center responded with the following: Staff #1 will obtain the work phone numbers for both parents and place it in the record.
