Inspector Notes
An unannounced inspection was completed on 12/20/2024 from 10:12 a.m. to 12:40 p.m. in response to a complaint received by the licensing office 12/12/2024 relating to allegations of building maintenance and forbidden actions. The inspector reviewed 1 staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/01/2025. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. The infant room was observed to have: the diaper changing table was caked with unidentified debris; multiple crib sheets were stained, yellowed, torn, and had an unidentified debris on them; carpets were stained, and there was debris on top of carpet, dirt and bug carcasses between carpets; and a live bug was observed on floor near where the children?s dining table and chairs.
Plan of Correction: Deep clean of building performed 12/28/2025. Staff mandated to follow daily cleaning checklist. Management will coach in the moment if any errors found.

The following actions or threats thereof are forbidden: threatening physical punishment. S1 acknowledged threating children with physical punishment ?Do I need to go get your bottom?, and then slapping her own legs making spanking sounds.
Plan of Correction: Staff will be coached in the moment if allegation of threatening language are used towards children.

The following acts or threats are forbidden: withholding food and drink.
S1 was observed by another staff withholding milk at breakfast from children because they were spilling it.
