During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was conducted on September 29, 2025. The on-site inspection began at 1:30 p.m. and ended at 5:15 p.m. The inspector reviewed compliance in the areas listed above. There were 73 children present and 12 staff. The inspector reviewed 7 children?s records and 7 staff records on-site . This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child 4, enrolled over a month ago, did not have documentation of an immunization record on file.
Plan of Correction: Immunization records will be obtained prior to a child's first day of enrollment.
Standard 8VAC20-780-240-C
Orientation must be documented for each staff prior to working alone with children and shall include specific topics. An old orientation form was used which excludes policies such as policies on allergic reactions, first aid and CPR, shaken baby syndrome, and SIDS. Staff 2, Staff 3, Staff 4, Staff 5, and Staff 6 did not have documentation of all required policies on the new staff orientation form.
Plan of Correction: The newest orientation form will be utilized to ensure all policies are reviewed and documented.
Standard 8VAC20-780-510-E
The center shall ensure that procedures for administering medication shall include methods to prevent use of outdated medication. The medication for one child expired
5/2025.
Plan of Correction: Medications that have expired will be sent home or destroyed.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the facility shall notify parents that that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. The medication authorization for Child A expired four months ago. The medication authorizations for two medications for Child B expired 6 days ago. The authorization had not been renewed and the medication was not returned or destroyed as required. The medication was on site.
Plan of Correction: Medications will be sent home when the medication authorizations expire.
Standard 8VAC20-780-550-F
Documentation shall be maintained of at least one lockdown drill annually. There was no documentation of a lockdown drill in the past 12 months.
Plan of Correction: Lockdown drills will be completed and documented annually.
Standard 8VAC20-780-560-F-4
Children three years of age or younger may not be offered foods that are considered to be potential choking hazards. In the infant, toddler, and twos room, children were served craisins as afternoon snack. Craisins are considered a choking hazard.
Plan of Correction: Craisins will not be served to children under the age of 3 years old.
Standard 8VAC20-780-560-M
No child shall be allowed to drink or eat while walking around. A child in the twos room was walking around drinking from a sippy cup.
Plan of Correction: Children will not be permitted to walk around and drink.
Standard 8VAC20-780-60-A
The facility shall have a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child 1, Child 2, and Child C have diagnosed food allergies but did not have a written allergy action plan on file.
Plan of Correction: Allergy action plans will be obtained for children with diagnosed food allergies.
