During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated on 10/3/2024 and completed on 10/8/2024. The on-site inspection began at 7:45am and ended at 8:15am. The inspector reviewed compliance in the areas listed above. There were 29 children present and 5 staff. The inspector reviewed 5 staff records electronically on 10/8/2024. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/5/2024. A POC submitted after this date will not appear on the public website.

The license and any other documents required shall be posted in a conspicuous place on the licensed premises.

Standard 8VAC20-770-60-C-2
For staff hired before July 1, 2024, providers must obtain the results of a central registry search for an employee, within 30 days of service.

Staff #3 had central registry results dated over 30 days after being employed.
Plan of Correction: Admin has communicated with HR about CRS timeline.
Standard 8VAC20-780-240-B
Staff are required to complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

Staff #2 and staff #5 completed orientation training more than 10 days after their first date of employment.
Plan of Correction: Director will ensure OC are reviewed within 7 days of hire date.
