Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview, and a virtual tour of the program.
A monitoring inspection was initiated on 1/19/2022 and concluded on 1/25/2022. The director/provider was contacted by telephone and a virtual inspection was conducted. There were 96 children present, ranging in ages from 1 year to 5 years, with 18 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Based on a review of staff records, 1 staff member did not have the results of a central registry background check for a state in which the individual has resided in the preceding five years.

Evidence:
1. Staff #1 (doh: 8/31/2021) did not have the results of a central registry background check for State #1 in which the individual has resided in the preceding five years.
Plan of Correction: We will conduct the out-of-state central registry as soon as possible.
