During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated on April 29 2025 and completed on April 29 2025 . The on-site inspection began at 10:00am and ended at 11:45am. The inspector reviewed compliance in the areas listed above. There were 101 children present and 28 staff. The inspector reviewed 9 children?s records and 8 staff records. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on May 7 2025. A POC submitted after this date will not appear on the public website

The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.

The date of the last annual fire inspection was listed as April 10 2025. The center is 2 weeks out of compliance.

After the first license, annual approval from the health department shall be provided for meeting requirements for:

When food is brought from home, the following shall apply: 1. The food container shall be sealed and clearly dated and labeled in a way that identifies the owner;

3 lunch boxes that contained food brought from home in one classroom was incorrectly dated. The lunches were dated April 28 2025. The date of inspection was April 29 2025.
