During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site code compliance inspection was initiated on 10/16/2025 and completed on 10/16/2025. The on-site inspection began at 11:20am and ended at 12:33pm. The inspector reviewed compliance in the areas listed above. There were 54 children present with 14 staff. The inspector reviewed 5 children?s records and 5 staff records on 10/16/2025. Information gathered during the inspection determined non-compliances with applicable code sections, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/24/2025 . A POC submitted after this date will not appear on the public website.

The required ratio for children ages 16 months to 24 months is one staff per five children.

One staff member was observed alone on the playground with 7 children (youngest child was 16 months) from approximately 11:25 a.m. to 11:30 a.m.
Plan of Correction: Remove barrier between playground areas. Retrain staff on ratios.

The required ratio for children ages 24 months to 36 months is one staff per eight children.

A staff member was observed alone on the playground with 12 children (youngest child was 2 years old) from approximately 11:25am to 11:30am.

The center is required to have employees undergo a background check every five years.

The most recent central registry check for staff #4 was overdue by one year.
