Inspector Notes

Today an unannounced Monitoring Inspection took place between the hours of 12:15pm and 1:30pm. There was a total of 17 children with 5 staff within the supervision guidelines. The children were observed eating lunch, playing games, and other organized activities. A complete inspection of the physical plant, staff records, and observations were made during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard 22.1-289.035-B-4
Based on a review of staff records, 1 out of 4 staff did not have the results of a search of the child abuse and neglect registry of equivalent for a state in which the individual has resided in the preceding five years.

Evidence:
1. Staff #4 (doh: 12/1/2021) did not have the results of an out-of-state background check for state #1 in which the employee has resided in the preceding five years.
Plan of Correction: We will follow-up on this since we sent it and and we will document the follow-up.

Based on a review of staff records, 3 out of 4 staff did not have the results of a central registry background check within 30 days of employment.

Evidence:
1. Staff #1's (doh: 11/22/2021) central registry results were dated 1/8/2022.
2. Staff #3's (doh: 12/1/2021) central registry results were dated 1/8/2022.
3. Staff #4's (doh: 12/1/2021) central registry results were dated 1/8/2022.
Plan of Correction: We will follow-up if central registries are not received within 30 days.
