Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 01/17/2025 and completed on 01/17/2025. The on-site inspection began at 11:45 am and ended at 1:55 pm. The inspector reviewed compliance in the areas listed above. There were 25 children present and 4 staff. The inspector reviewed 6 children's records on-site and 5 staff records on-site on 01/17/2025.

The child day center shall obtain a copy of the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

The results of a central registry check was not on file for staff 1 who has resided in a state outside of Virginia within the past five years.
Plan of Correction: Administrative staff stated that a central registry check will be obtained for staff 1 today.
