Inspector Notes

An onsite code compliance inspection was conducted October 10, 2023. The inspection was initiated at approximately 10:30 AM and concluded at approximately 1:00 PM. There were 59 children present, ranging from 12 months to five years of age, with 18 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of six children's records and six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.031-A-4
Based on review of six staff records and interview, the center did not ensure two new staff have been certified by a practicing physician to be free from any disability which would prevent him from caring for children by the first date of employment.
Evidence:
1. The record of Staff #3 (Date of Employment 12/12/2022) contained a Staff Health Report form dated 09/22/2023.
2. The record of Staff #4 (Date of Employment 09/05/2023) contained a Staff Health Report form dated 09/22/2023.
3. A member of management confirmed this information during the inspection.
Plan of Correction: We will ensure staff health reports are obtained prior to staff beginning employment.
