Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1 was missing the address for at least one person designated by the parent to contact in case of an emergency and the parent cannot be reached.

Standard 8VAC20-790-540-B-11
A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #4 has a diagnosed food allergy and did not have a written care plan in their file.

All staff who work directly with children shall have within 90 days of the date of employment or 90 days from subsidy vendor approval a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care. The training shall include an in-person competency demonstration. The documentation in the records Staff #2, Staff #4, Staff #5, Staff #6, and Staff #7 showed CPR certification expired in September 2024.
