Inspector Notes
An in person monitoring study was conducted today. Emergency supplies, emergency evacuation drills, infant bottles were labeled/dated, cot spacing, crib labeling were all in compliance. Violations were discussed with the Director at the exit interview.
Children were observed participating in a variety of activities, including handwashing and eating lunch. The playground was not inspected due to inclement weather.
LI was present today from 10:30 AM ? 12:49 PM.
The inspection will be typed off site and emailed to the facility for review and signatures.

11There is no documentation of a completed Central Registry Search within 45 days of employment.
Evidence:
There is no completed Central Registry Search, Criminal History Check or Sex Offender search for 2 residences outside of Virginia in the past 5 years. (Staff C- DOE: 7/15/21)

The SDS on file does not accurately reflect all states or US cities/territories of residence outside the state of Virginia in the past 5 years. There were 2 residences outside of Virginia. (Staff C- Date of employment (DOE): 7/15/21)
Evidence:
One staff member, in a lead teacher position, does not have enough college credits to meet lead teacher qualifications. Available documentation shows 32 of the 48 required credit hours. (Staff A)
Evidence: The School Age room had a spray bottle containing bleach water in a cabinet that was not locked.

*The infant room ratio is 4:1 children to staff. At approximately 12:30 PM. There were 7 infants and 1 staff present.
Evidence:
The Two?s room had a Toddler present in the room. There were 8 children with 1 staff. The applicable ratio was 5 children to 1 staff.

4 of 4 infants that require tummy time did not have any of the required 30 minutes of tummy time documented on 10/4/22.

At approximately 12:30 PM 5 infants were observed sleeping in bouncy seats with blankets covering the children.

One staff has been terminated from the facility. The staff record does not contain documentation of the date of separation of employment. (Staff B)

*The infant room staff stated there was no attendance sheet kept.
*The attendance sheet for the 2?s room did not reflect a Toddler in the room.
*The School Age (SA) room attendance sheet reflected 10 children; however, there were 12 present. 2 children were on the Preschool side of the room.
