Inspector Notes
An unannounced monitoring inspection was conducted today.
All rooms were in ratio during the inspection. Children were resting quietly during the inspection.
3 child and 2 staff records were reviewed. See Violation Notice for non-compliances cited today.
Emergency drills, emergency supplies, first aid kits, medications, cot spacing, top and bottom covers for rest cots, infant bottles labeled/dated were all in compliance today.

history search for a residence outside of Virginia within the past 5 years, prior to employment.
Evidence:
Staff A's record did not contain a completed criminal history search for a residence within the past 5 years, outside of Virginia.
Based on a review of records, one staff record contained documentation of a completed Sworn Disclosure Statement (SDS) that did not accurately reflect residences outside of the state of Virginia in the past 5 years.
Evidence:
Staff A's SDS did not disclose a second residence outside of Virginia within the past 5 years.
