Inspector Notes
An unannounced monitoring inspection was conducted today.
All rooms were in ratio during the inspection. Most children were resting quietly during the visit.
2 staff records were reviewed, one for specific training only.
2 child records were reviewed. Both were complete.
Medications, cot spacing, top and bottom covers for resting children, bathroom supplies, emergency supplies, first aid supplies, emergency evacuation drills and the menu were all in compliance.
Violations were reviewed with the Administrator in charge.

Standard 8VAC20-780-40-M
Based on observation, the center did not maintain a written list of all children's allergies in each room where children are present.
Evidence:
There was no allergy list in the School-Age and Preschool rooms.
Plan of Correction: There was an allergy list in all classrooms. The School Age allergy list have been updated as there are no allergies in that class. The school age and 4 years share a classroom and allergy list.
Standard 8VAC20-780-500-B
Based on observation, a foot operated diaper disposal container was not used in the infant room to dispose of a soiled diaper.
Evidence:
A staff member was observed placing a soiled diaper on a diapering table instead of into a foot operated disposal container.
Plan of Correction: The Director has reviewed diaper changing procedures with all staff during the Weekly meeting.
Standard 8VAC20-780-550-P
Based on a review of injury reports, all required information was not recorded.
Evidence:
2 of 3 reports reviewed did not include the time of parent notification.
Plan of Correction: Staff is required to be sure accident reports are completely filled out. Staff was reminded of this during the 4/23/24 Weekly meeting.
Standard 8VAC20-780-570-A
Based on observation, an infant was placed in an seat with a safety belt not in use.
Evidence:
An infant was observed in the infant room in an infant seat, with no safety strap buckled.
Plan of Correction: Safety is important and staff were given a verbal warning in the matter. Going forward staff is to follow the safety procedures and be sure safety belts are always used when children are in a seat.
Standard 8VAC20-780-80-A
Based on a review of documentation, an accurate attendance sheet was not maintained for each group of children.
Evidence:
The preschool room had 13 children present; however, there were 12 children marked on the attendance sheet.
Plan of Correction: We have added the 12th child to the Pre-school attendance instead of the school age attendance, although the child attends public school full time. This will improve the accuracy of the attendance book.
