During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/16/2025, as a part of the licensure period. The on-site inspection began at 10:11 AM and ended at noon. The inspector reviewed compliance in the areas listed above. There were 36 children present and 13 staff. The inspector reviewed 3 children?s records and 3 staff records on-site. This inspection included: document review, tour of the facility, interviews and observations
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/29/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-240-C
Orientation training shall be appropriate to the age of the children in care and include recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse as required by ? 63.2-1509 of the Code of Virginia.
Staff A's record did not contain documentation of completing Child Abuse and Neglect training. Documentation was overdue by 4 months.
Plan of Correction: Staff member was asked to complete the training during the inspection, and was completed 10/20/2025.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days. Medications that are not picked up by the parent within 14 days will be disposed of by the center.
One medication at the center was not disposed of after 14 days of parent notification, to pick up the medicine.
Plan of Correction: Medication was disposed of during the inspection.
