Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety (SHSI) inspection was initiated and concluded on 9/21/2021. There were 58 children present with 8 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Standard 22VAC40-665-540-A
Based on review of five staff records, the center did not have documentation of the tuberculosis (TB) test or screening that had been completed within the last 30 calendar days of the date of employment.
Evidence:
1. Staff #4 (date of hire 2/22/2021) had a tuberculosis test or screening dated 11/25/2020 and was more than 30 calendar days of the date of employment.
2. Staff #5 (date of hire 6/21/2021) had had a tuberculosis test or screening dated 3/16/2021 and was more than 30 calendar days of the date of employment.
Plan of Correction: For any new hire, we will have them get a new TB test regardless of the date.
