Inspector Notes

A self-reported incident was received by the department on November 30, 2023, regarding a staff member grabbing a child by the shoulder, squeezing, and forcing the child to sit down on the cot. Interviews were completed with staff and children over three days on 12/05/2023, 12/06/2023, and 12/07/2023.

The information gathered during this investigation of the self-reported incident determined non-compliance with applicable standards or law and one violation was documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.

Based on interviews, behavioral guidance was not constructive in nature, age and stage appropriate, and intended to redirect children to appropriate behavior and resolve conflicts.
Evidence:
1. On 11/29/2023 during nap time, Child #1 (4 years old) got up from the cot to go to the bathroom. Staff #1 yanked his arm and pulled him with both hands by the shoulder and pushed him back on his cot.
2. Another child stated when Child #1 doesn't listen to Staff #1, she pulls Child #1's arm and puts him in time out.
3. A staff member stated that she sometimes hears Staff #1 yell at the children in Staff #1's classroom.
Plan of Correction: Management has reviewed and retrained all staff on proper child
guidance procedures.
