Inspector Notes

An unannounced inspection was initiated on and completed on 8/07/2025 in response to a complaint received by the licensing office on 8/05/2025 relating to allegations of building maintenance, and preventing the spread of disease. This inspection included document review, ?
tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to building maintenance and hazardous substances and other harmful agents and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 8/14/2025. A POC submitted after this date will not appear on the public website.

Repeat Violation
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

The paint was chipping in large areas near the wood blocks and to the left and right side of the door that exits outside in the Club mates classroom.
The top of the door on the play refrigerator was missing.

Hazardous substances must be in a locked place that prevents access by children.

In the Preschool 2 Classroom, 4 containers of disinfectant sprays were inside a container that was labeled cleaning supplies on a high shelf in the bathroom and were not kept locked.
Plan of Correction: Cleaning supplies were put away locked in cabinets.
