Inspector Notes

An unannounced, on-site code compliance inspection was initiated on 8/18/2025 and completed on 8/18/2025. The on-site inspection began at 11:14AM and ended at 1:25PM. The inspector reviewed compliance in the areas listed above. There were 68 children present with 16 staff. The inspector reviewed 3 children?s records and 3 staff records on site. This inspection included - document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.

Each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. Staff #3, who has been employed for two years, annual staff health report was overdue by 6 months.
Plan of Correction: Provider will update annual health reports.
