Inspector Notes
An unannounced monitoring inspection was conducted on August 30, 2023 from approximately 11:15 am to 1:15 pm. There were 83 children in attendance and a total of 21 staff present. 8 staff records and 8 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and playgrounds. The children were observed getting diapers changed, eating lunch, and taking a nap while the licensing inspector was on-site.
Violations were cited as a result of this inspection.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign and return it to me within 5 business days from today, 09/15/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

6Evidence: The record for staff #7, date of employment 03/28/2023, did not contain the results of a central registry finding. Interview with administration confirmed that the record did not contain the results of the central registry finding. Staff #7 was observed working in a classroom with children.
Evidence: When asked to view the annual fire inspection report the Director stated that they did not have a copy of the most recent fire inspection report. The center reached out to the fire official while the Licensing Inspector was on site and determined that the most recent fire inspection was completed on 08/25/22. Another fire inspection was due on or before 08/25/23.
Evidence: Classroom #4 was observed to have 2 staff with 17 children age 2 years old. Staff #2, confirmed that there were 17 children in attendance. The daily attendance sheet had 17 children signed into classroom #4.
Based on documentation review, and interview, the center failed to implement a monthly practice evacuation drill.
Evidence: Documentation review revealed that evacuation drills were not conducted in the months of May, June, or July 2023. Administration confirmed that evacuation drills were not conducted in the months of May, June, or July 2023.
Evidence:
1. The record for staff #6, date of employment 2/1/23, did not contain documentation of the required Virginia Department of Education sponsored orientation course.
2. The record for staff #7, date of employment 3/28/23, and the record for staff # 4, date of employment 8/3/23, did not contain background checks as required.
