Inspector Notes
An unannounced renewal inspection was conducted on March 1, 2024 from approximately 10:00 am to 1:15 pm. There were 84 children in attendance and a total of 19 staff present. 10 staff records and 8 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed reading a story, getting diapers changed, making paper lions, playing outside, coloring, playing with blocks, eating lunch and taking a nap. while the licensing inspector was on-site.
Violations were cited as a result of this inspection.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today, 03/26/2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).
Violations
3Based on record review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening. The documentation shall have been completed within the last 30 calendar days of the date of employment.
Evidence: The record for staff #4, date of employment 01/03/24, contained a tuberculosis screening dated 07/31/23.
Evidence: The record for staff #2, date of employment 03/08/2022, contained documentation of 12.5 hours of annual training from January 2023 to January 2024.
Based on observation, the center failed to ensure that children's hands shall be washed with soap and running water after toileting.
Evidence: Staff #4 was observed changing a child's diaper. The child's hands were not washed after the diaper change.
