Inspector Notes

An unannounced, on-site focused inspection was initiated on 08/13/2024 and completed on 08/20/2024, as a part of the provisional licensure period. The on-site inspection began at 10:45 am and ended at 1:30 pm. The inspector reviewed compliance in the areas listed above. There were 110 children present and 17 staff. The inspector reviewed 8 children?s records and 4 staff records on-site and electronically on 08/20/2024. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Based on observation, and interview, the center failed to ensure that prepared infant formula shall be refrigerated, dated, and labeled with the child's name.
Evidence: The refrigerator in classroom #1 contained 2 bottles with prepared infant formula that were not dated or labeled with the child's name. Staff in classroom #1 confirmed that the 2 bottles were not dated or labeled with the child's name.
