Inspector Notes

An unannounced inspection was initiated on 08/13/2024 and completed on 08/20/2024 in response to a complaint received by the licensing office on 08/12/2024 relating to allegations of transitions and diaper changing procedures. The inspector reviewed 1 child?s record and 1 staff record on-site. This inspection included document review, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/29/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
Based on document review, and interviews, the center failed to ensure that when staff are supervising children, they shall always ensure their care, protection, and guidance.
Evidence:
1. Child #1 arrived at the center at approximately 10:13 am.
2. Child #1 was moved from the child's assigned classroom to another classroom due to a staffing shortage.
3. Documentation review revealed that child #1 received a diaper change at approximately 10:24 am.
4. Interviews revealed that child #1 did not receive another diaper change on 06/05/2024.
5. Interviews revealed that child #1 developed a diaper rash and infection which required a doctors appointment and medication.

Standard 8VAC20-780-40-E
Based on documentation review, and interview, the center failed to follow the center's own policies and procedures.
Evidence:
1. The KCE Brandermill Diapering policy training states that "every child in our care, regardless of age, that wears diapers or pull-ups will be changed every 2 (two) hours or as soon as the teacher notices that they have soiled the diaper regardless of wet or bowel movement to ensure proper hygiene."
2. The KinderCare app indicated that on 06/05/2024 child #1 had a diaper change at 10:34 am. The app did not notate any other diaper changes occurring on 06/05/2024.
3. Interview and written statement from staff #1 determined that child #1 did not have any other diaper changes on 06/05/2024 after the 10:34 am diaper change.
4. Child #1 was picked up from the center at approximately 3:37 pm.
