Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

This inspection was amended on 03/16/2026.
An unannounced, on-site monitoring inspection was initiated on 03/03/2026 and completed on 03/12/2026. The on-site inspection began at 11:12 am and ended at 2:45 pm. The inspector reviewed compliance in the areas listed above. There were 90 children present and 14 staff. The inspector reviewed 19 children?s records and 9 staff records on-site and electronically on 03/05/2026. This inspection included document review, tour of the facility, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 03/20/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-240-A
Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.

Staff #1, employed for 7 months, completed the course 5 months late.
Plan of Correction: New hires are now expected to complete the 10 hour preservice prior to their start date. This expectation has gone into effect with our recent new hires.

The required ratio for children ages birth up to 16 months is one staff per 4 children.

1. Staff #2 and staff #8 were observed supervising 10 children (youngest child 5 months) in classroom #1 from approximately 11:12 am to 11:35 am. The face to name tracking sheet documented that staff #2 and staff #8 were supervising 10 children (youngest child 5 months) for approximately 2 hours.
Plan of Correction: As will the 16 to 24 months students, the infant students will be sent home in the event that the classroom becomes out of ratio and no support is available. The teachers will also undergo training in ratios and CSR best practices with knowledge of knowing when to ask for help to comply with state licensing standards.

The required ratio for children ages 16 to 24 months is one staff per five children.

Staff #4 and staff #6 were observed supervising 11 children ages 16 to 24 months (youngest child 17 months) from approximately 11:12 am to 11:35 am. The face to name tracking sheet documented that staff #4 and staff #6 supervised 11 children ages 16 to 24 months from approximately 9:30 am to 11:34 am.
Plan of Correction: Children will be sent home in the event of possibly being out of ratio. If a teacher from a sister center is available, one will be asked to help our center. All staff members will be trained continuously in ratios and CSR best practices with the knowledge of knowing when to call management for help to comply with state licensing standards.
Standard 8VAC20-780-420-E-3
Staff shall request at least annually parent confirmation that the required information in the child's record is up to date. Up-to-date information is required to be requested from the parent for each child's record at least annually.

1. The information in the record of child #1 had not been updated in over 16 months which is overdue by 4 months.
2. The information in the record of child #2 had not been updated in over 3 years which is overdue by 2 years and 6 months.
3. The information in the record of child #4 had not been updated in over 2 years which is over due by 15 months.
4. The information in the record of child #7 had not been updated in over 15 months which is over due by 3 months.
Plan of Correction: We will have a mass email sent to families requesting updated records for all students. The families of the students with the out of date documentation will be notified. After the mass email to our families requesting updated documentation, there will be a schedule made. The schedule will be a reminder of when it is time to audit all files.

Lockdown procedures shall be practiced at least annually.

The center did not practice lockdown procedures in 2025.
Plan of Correction: We are now aware of the requirement for lock down procedures. One will be conducted on a yearly basis.
