Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 6/3/25. The on-site inspection began at 10:10 am and ended at 12:20 pm. The inspector reviewed compliance in the areas listed above. There were 24 children present and six staff. The inspector reviewed five children?s records and one staff record on-site. This inspection included: document review, tour of the facility, interviews,
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.
Staff #1, employed for two months, did not have documentation of requesting a central registry check or out of state sex offender request from one state.
Plan of Correction: The director will ensure that all out of state background checks are conducted prior to hire date.
Standard 8VAC20-780-70
Documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position. Information about any health problems that may interfere with fulfilling the job responsibilities are to be kept at the center.
Staff #1 has been actively working at the center the past two months, did not have documentation of a diploma or transcript on file. Staff #1 also did not have information about any health problems that may interfere with job duties on file.
Plan of Correction: The director will add this question to the application and will ensure that all staff submit a diploma or transcript at time of hire.
