Inspector Notes
An unannounced monitoring inspection was conducted today from 10:15am-12:00pm. There were 89 children (2-5yrs.) directly supervised by 14 staff. The physical plant, 3 staff records, 3 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books and playing outside. There was an adequate supply of materials for the children. Areas of non-compliance are identified on the Violation Notice.

Violations
5Evidence:
1. The boy's bathrooms had large areas of the stall walls covered with rust. The rust was rough to the touch and could cut children's clothing and/or skin.
2. A broken soap dispenser was observed in the boys? bathroom located on the lower level of the facility.
3. Three tiles were missing on the second floor hallway next to the water fountain.
Evidence:
Four containers of disinfectant Clorox wipes were observed being stored on a shelf inside a closet in the Little Tykes classroom # 111. The door to the closet had a lock, but the closet door was not locked during the inspection.
In the Little Tykes classroom # L21, 4 containers of fluid absorbent were observed in an unlocked upper cabinet.
Evidence:
1. At approximately 11:15am there were 13 children present in the Preschool classroom (room #204) with Staff #1 supervising.
2. A child needed to use the restroom, Staff #1 called out for one of the teachers in the classroom located next to them, but there was no answer.
3. Then Staff #1 stepped out of their classroom, walked over to the other classroom and asked one of the teachers to take the child to the restroom.
4. As a result, the children were out of sight and sound supervision for aproximately 10 seconds.
Evidence:
1. At approximately 11:00am, 2 out of 7 classrooms were out of the required ratios.
2. A ratio waiver request was granted on 12/3/2021 increasing the ratio in the Little Tykes classroom (2 years-old) from 1 staff to 8 children to 1 staff to 9 children and in the Preschool classrooms (4 years-old) from 1 staff to 10 children to 1 staff to 11 children.
3. On the day of the inspection the following ratios were observed:
A) 19 children (2 years-old) were observed in the Little Tykes classroom #111 with 2 staff.
B) 13 children (4 years-old) were observed in the Preschool classroom #204 with 1 staff.
EVIDENCE:
Child #1's did not include a current food allergy care action plan. The allergy care plan for Child #1 expired on 3/5/2022.
