Inspector Notes

An unannounced renewal inspection was conducted today from 10:02am-12:10pm. There were 78 children (23months - 5 years) directly supervised by 15 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, coloring and eating snack. There was an ample supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training.

Standard 8VAC20-780-130-E
Based on record review, the center did not obtain documentation of additional immunizations once every six months for a child under the age of two years.

Evidence:
Child #1's record contain immunization records that were last updated on 7/14/2020.
Plan of Correction: We will update immunizations for those needed.
Standard 8VAC20-780-350-C
Based on observation, a ratio of 1 staff to 5 children was not maintained in a classroom where the youngest child was 23 months of age.

Evidence:
At approximately 10:02am, Staff #1 was observed on the playground. When the inspector entered the two's classroom at approximately 10:11am, there were 10 children with Staff #2. Staff #1 returned to the classroom at approximately 10:18am. The classroom was out of ratio for approximately 16 minutes.
Plan of Correction: We will monitor ratios throughout the day and switch staff when necessary to keep ratio.

Based on record review, interview, and observation, the center did not operate within the terms of their current license issued by the department.

Evidence:
The center is currently licensed for children ages 2 years- 12 years of age. Child #6 was enrolled in the center on 09/05/2023, they were 21 months of age.
Plan of Correction: We will not enroll children under 2 years old.
