22.1-289.031 Religious Exempt

During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated and completed on 5/20/25 with a local agency, in response to a complaint received by the licensing office on 5/16/25 relating to an allegation regarding supervision. This inspection included interviews. The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with an applicable standard or law and a violation is documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/27/25. A POC submitted after this date will not appear on the public website.

The center is required to establish and implement procedures for appropriate supervision of all children in care.

On 5/14/25, at approximately 11:00am, Staff #1 and Staff #2 transitioned their class outside to the playground where the children played for about 10 minutes. Upon returning to the classroom, the staff members found Child A, five-years-old, in the classroom, and the child stated that they had been left alone. The program?s procedure was to do a headcount prior to leaving the classroom. Staff #1 reported doing a headcount but miscounting due to the children moving around. Staff #2 reported they did not know to do a headcount.
Plan of Correction: Addendum to Handbook (extra safeguards to ensure proper supervision)- New required practice for ANY classroom departure:
-Line up students and rollcall by name immediately before departure
-Lead teacher should be at one end of the line and aide at the other
-Cross check rollcall by counting from both ends of the line
-Confirm count with each before moving to your next location
-Teacher in the back of the line is responsible for watching the line move out the door
-Once last student has crossed into the hallway that teacher MUST perform a "sweep" of the classroom and bathroom
-Another class count should be done upon arriving to your new location
-When leaving to go back to your classroom the same process MUST be followed starting with a rollcall by name

This process of departure will be used beginning Thursday, May 15, 2025. It will be the only metho used for moving back and forth to various locations on campus. Additional headcounts are certainly welcome but there is not to be less than the above stated protocol.

This practice is to ensure the supervision of all students at all times. If a student moves to a different spot in line or leaves the line, the process begins again.
