Inspector Notes

An unannounced code compliance inspection was conducted on May 2, 2024 between the hours of 11:00am - 12:30pm. There were 52 children present, ranging in ages from two to five years, with 8 supervising staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision and procedures. A total of 6 children?s records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice. Based on record review and interview, the child day center failed to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
1) Staff # 1 (date of hire: 08/2023) did not have a record of a criminal history record information check, a sex offender registry check, or a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years on file.
2) Staff # 6 confirmed that the center did not run the three out of state background checks required for Staff # 1.
