Inspector Notes
A code compliance inspection was conducted on Tuesday, May 9, 2023. The inspector was on site from 9:30am to approximately 11:35am. There were 140 children in attendance with 24 staff directly supervising. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. The center is equipped with toys and supplies and items were available to the children. All areas of the facility including classrooms, hallways, the outdoor play area, and bathrooms were inspected. Ten children's records and ten staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

3Evidence: 1) The staff health report in the record of Staff #3, employed on 09/01/22, was completed on 09/09/22.
2) The staff health report in the record of Staff #6, employed on 09/01/22, was completed on 09/21/22.
The staff health report for each staff member should have been obtained prior to the staff beginning employment.
Evidence: 1) The fingerprint-based national criminal background check in the record of Staff #2, employed on 09/02/21, was completed on 09/15/21.
2) The fingerprint-based national criminal background check in the record of Staff #4, employed on 09/13/21, was completed on 10/08/21.
3) The fingerprint-based national criminal background check in the record of Staff #5, employed on 08/26/21, was completed on 03/29/21. The facility must not accept a required background check that is dated more than 90 days prior to employment.
5) The fingerprint-based national criminal background check in the record of Staff #10, employed on 07/22/21, was completed on 10/13/20. The facility must not accept a required background check that is dated more than 90 days prior to employment.
6) During interview, a member of management acknowledged the required background checks were not obtained within the required timeframes.
