Inspector Notes

An annual RECDC health and safety inspection was conducted today from 9:30am-10:35am. The center had 43 children in care with 9 staff within the supervision guidelines. A tour of the facility was conducted, records were reviewed, and observations were made. The children were
observed creating art projects, participating in dramatic play, handwriting, and reading books. Violations were found during today's inspection and are listed on the Violation Notice. Thank you

Based on a review of staff records, the child day center did not obtain a copy of the search of the child abuse and neglect registry or equivalent registry from any state in which an employee has resided in the preceding five years.

Evidence:
1. Staff #1 (doh: 9/1/2022) did not have documentation of an out-of-state child abuse and neglect central registry search from 2 different states in which the employee lived in during the preceding five years.
Plan of Correction: I will send both background check requests out right away.
